The following items should be completed and submitted to the Graduate Council to request approval for a new graduate course offering.
1. Request for New Course
Number form (issued by the Assistant Registrar for Administrative
Services for Academics;
2. New Graduate Course Application
form revised 8/01, copy attached;
3. Cover memorandum signed by academic department head/director
and college dean (see example);
4. Undergraduate Curriculum Committee approval is required
if a dual-level course. This information should be indicated
in Item #1 of the New Graduate
Course Application form.
NOTE: Please furnish Graduate Council with 15 copies
of each document listed in the above items.
