1. Determine if you applying for an on-campus degree or the online degree completion program.
2. Download and complete the application: Dental Hygiene Application .pdf
3. Return your application to the Dental Hygiene department (on-campus degree program)
or the GOLD program (online degree completion program).

Visit the Gateway To Online Degrees (GOLD) site to apply for the Dental Hygiene Online Degree Completion Program.
Please note the Online Degree Completion Program has a different and separate admissions and
application process.
