Student Complaint Procedure

Department of Health Studies Complaint Procedures

 

The Department of Health Studies at the University of Louisiana at Monroe has in place the following procedures for addressing student complaints*: 

 

Step 1:  The student may make an inquiry regarding complaint procedures or about issues and concerns that could be considered complaints with any Department of Health Studies faculty member.

 

Step 2:  If the student feels there is a legitimate complaint, he/she will initiate a meeting at the class level with the instructor of record to attempt to resolve the issue.

 

Step 3:  If the student feels there was no satisfactory resolution of the issue at the class level, the student may request a meeting with the Department Head of the Department of Health Studies.

 

Step 4:  If the student feels there was no satisfactory resolution of the issue at the Department Head level, the student may file a formal complaint with the Dean of the College of Health Sciences.  The complaint will not be processed unless the following steps are followed: 

A. The formal written complaint must be filed in writing within 30 days after meeting with the Department Head.

B. The written complaint must have an original signature by the complainant.

C. The written complaint must state the nature of the complaint in five sentences or less.

D. The written complaint must briefly describe the details of the complaint in the clearest possible language.

E. Materials and documentation used to support a complainant’s allegations should be directly related to the reported case and included with the written complaint.

F. The written complaint must indicate the time frame in which the violation referenced in the complaint occurred.

G. The written complaint must describe the steps taken to date to exhaust the Department of Health Studies' complaint procedures and the Department’s response to the complainant as a result of those procedures.
