At the start of the semester prior to your internship, you should have:

(a) contacted potential internship site(s); in most, but not all, cases we will be able to grant your first choice, but you should have at least one back-up site in mind.

(b) discussed with the potential site supervisor tentative project ideas that will be completed during the internship; the project should take roughly 30-40 hours of your internship to complete, but you may use time at the site or independently to complete this project

(c) signed your Hepatitis B Vaccination Waiver Form

(d) signed your Health Insurance Policy Waiver Form

(e) purchased liability insurance that will carry throughout the complete internship experience.  Various options are available through one of the sites at the bottom of this page

ACSM: Discount insurance is available to current ACSM Members and ACSM Certified Professionals.  Forrest T Jones, Inc. is the insurance company that represents ACSM.  They do not share rate information with ACSM.   Thus, you will need to contact them directly.  When calling, please identify yourself as an ACSM Member or an ACSM Certified Professional (whichever is applicable.)  You may reach Forrest T Jones at 1-866-820-5183.

(h) secure a contract from the Kinesiology Department to obtain a signature from the site supervisor or secure one from the site to be reviewed by the ULM Department of Sponsored Programs and Research for approval.  The typical pattern is for the site to sign the contract after Dr. Doherty, Dr. Colvin, and, finally, Dr. Richters sign it.  A copy is sent to the site and the original is kept on file in the main KINS office.  In some instances, the site will send their standard contract, signed, and ask for signatures from the ULM representatives.  In most cases, either is acceptable.

NOTE: Wherever you see "University Internship Coordinator" in the following documents it means the Kinesiology faculty member teaching the internship, not the ULM Internship Coordinator, Ann Smith.

These documents must be in the Kinesiology Department by your first day of the internship.  It is strongly recommended that you contact the ULM internship coordinator, which may vary from semester to semester, to ensure the site is acceptable.  Call 342-1306 to find out who the ULM internship supervisor will be for your planned semester(s).
