Summer I/II 2009 and Fall 2009 Interns...

Spring 2010 Interns...

Mid-way through the fall semester, you should have:

(a) contacted potential internship site(s); in most, but not all, cases we will be able to grant your first choice, but you should have at least one back-up site in mind.

(b) discussed with the potential site supervisor tentative project ideas that will be completed during the internship; the project should take roughly 30-40 hours of your internship to complete, but you may use time at the site or independently to complete this project

(c) had a basic physical completed at the ULM Health Center, local clinic, or your own physician

(d) signed your Hepatitis B Vaccination Form

(e) signed your Health Insurance Policy Form

(f) purchased liability insurance through one of the sites at the bottom of this page

(g) if you have not completed a criminal background check within the last year, stop over the ULM Police to see Lt. Johnson WHEN TOLD TO DO SO by Dr. Doherty.  Lt. Johnson who will fingerprint you for your criminal background check and you should not have to pay additional money to do so. Your class fee pays for this service.  Bring your completed fingerprints to the main KINS office and fill our the Criminal Background Request form, which Ms. Sonya has.

(i) Once you receive your criminal background check in the mail at whatever address you put on the Criminal Background Request form, which should be sometime prior to the start of your internship, but may actually be shortly after it begins due to delays that are out of our control, you should deliver it to the Kinesiology Department.  It will remain securely on file in the main KINS office until such time as your academic folder is shredded.

NOTE: Wherever you see "University Internship Coordinator" in the following documents it means the Kinesiology faculty member teaching the internship, not the ULM Internship Coordinator, Ann Smith.
