To complete your application process, submit the following:

1. ULM Collegiate Program Application

2. Submit $20 Application Fee (check or money order)

 

3. Recommendation from high school principal, with principal’s original signature*

4. Proof of Immunization Complainace Form (if medical records are not current, complete the top section on the form and the exemption section at the bottom, giving a reason for the exemption.)

5. Copy of Social Security card

 

6. Class Selection Form with guidance counselor's AND parent(s) original signature*

6a. List of Suggested Courses: for student reference only

7. Official ACT or SAT scores

 

8. High School Transcript

 

9. Transcript Request Form to request your transcript be sent to your high school upon completing your coursework
