Hazard
control logs are a way that employees & students can report safety
concerns, issues, and hazards anonymously. The hazard
control
log form will be posted in all buildings with the other relevant safety
postings.
Employees
who complete the log should fill out the sections on date, location,
and
description of the hazard. If any temporary / immediate
corrective
action was taken by the employee this should be listed here also.
The long - term solution and scheduled date of completion will be
filled
out by the building safety coordinator and/or the environmental health
& safety officer.
Building
safety coordinators are responsible for checking logs weekly to see if
any hazards have been reported. If any hazards are reported, then
the building safety coordinator will arrange for corrective actions to
begin so that the hazard can be corrected. If deemed appropriate,
then corrective action must be initiated as soon as possible or at
least
within thirty days.
As
necessary
each hazard control log should be replaced with a new log. The
old
log should be mailed to the environmental health & safety office.
Hazards
requiring immediate attention that pose an imminent threat should be
reported
by telephone as soon as possible to the physical plant, 342-5170 and
the
environmental health and safety office, 342-5177.
If
corrective
action is not taken within 30 days after each hazard is discovered,
then
anyone can report the hazard to:
