ONLINE FORMS
Below are links to web pages from where various forms developed for the Controller's Office can be downloaded. Most, if not all, of the forms require signatures and, therefore, CANNOT be electronically submitted.
Controller and Student Online Forms are located in a secure directory only accessible on campus. If you have trouble accessing a form or are off campus, please contact the Controller's Office at 318-342-5122, or by e-mail, and alternative arrangements can be made. For the convenience of faculty/staff and students located off campus, some unsecured forms are listed which can be accessed from any computer.
The format of each form/document is indicated by the extension link. In order to view, fill in, or print the form(s), you must have the following software package(s) loaded on your computer: MS Word® for .DOC forms, MS Excel® for .XLS forms, or Adobe Reader® for .PDF forms.
